What Is Liposuction?

Liposuction is a procedure used to contour the body and remove areas of stubborn fat deposits that don’t seem to change despite diet and exercise, but it IS NOT a weight loss procedure.

Liposuction may be performed on almost any area of the body, although some areas are more difficult to treat. Once the fat cells are removed, they do not grow back. However, since human fat cells grow in size but not in number, you can still have fat accumulation in areas of prior liposuction, should you gain weight.

Liposuction is a very popular procedure. It’s been around for a very long time, but it’s also changed a lot over a period of time. I like to tell people right now that we really have some really great options for liposuction that we didn’t have say 10, 15 years ago. One of the tools that I use for liposuction is called VASER ultrasonic liposuction. It’s a product that uses a probe, which emit sound waves and helps break up the fat. Those sound waves actually generate some heat to help tighten and shrink the skin. That’s something that’s been around for a while, but it’s a great product. Liposuction essentially is adding a fluid to the fat to help shrink some blood cells or blood vessels that would bleed and then use a vacuum to remove the fat. It is a very artistic procedure, because you have to be able to sculpt that person. If you over resect, then they have a divot or a dent, and if you under resect, obviously they don’t get a good result.

It really is something that takes in artistic eye, and it’s very precise, and it’s something that I really enjoy doing. I like using these modern tools, such as the VASER ultrasonic liposuction. There’s also a device called power assisted liposuction. That’s a great device too. It makes the removal of the fat much easier. There’s some very specific techniques that are used. One of them is it’s the acronym SAFE. People can look that up on the web, but it’s called SAFE liposuction. It uses specific what we call cannulas to infiltrate the fat, to help break up the fat, and to help remove the fat, and then to help blend the remaining fat into a nice, smooth, even layer. Even though liposuction’s been around a long time, I would not want someone to do liposuction on me who was doing it the same way they did it 20 years ago. Times have changed, and there’s some really great techniques that decrease the risks and complications. I’m proud to say we have those available for you.

Who is the Ideal Candidate for Liposuction?

The best candidates for liposuction are healthy non-smokers who have a positive outlook and realistic goals in mind. At the time of your consultation, your goals, the procedure, and the routine post-operative healing process will be explained in detail, and all of your questions will be answered.

Good results from liposuction depend on three events:

  • Fat removal
  • Formation of scar tissue
  • Ability of your skin to be elastic and shrink

The ability of skin to be elastic and shrink to accommodate the removal of fat is imperative for good results. This is helped by wearing a compression garment after surgery. Areas of skin with lots of stretch marks will not shrink well after surgery. The ability of your skin to shrink is somewhat age-related and is not always predictable. Techniques to stimulate skin shrinkage may be required to improve results.

This is why wearing a compression garment over the treated liposuction area several weeks after surgery is necessary. Cellulite in women is due to particular arrangements of the fat cells. It is found in certain genetically predetermined areas of the body. The posterior thigh and buttock area are the most common regions.

There is no long-term improvement of cellulite from traditional liposuction. If fine, small cannula techniques are used with superficial suction, the cellulite appearance may or may not be improved.

During your consultation for liposuction we want to examine the areas that bother you. And the areas that we can liposuction are really, pretty much, the whole body. So we can address underneath the chin, we call the submental. We can do a little of the neck to help define the jaw line. We can do chests, we often do male chests. We can do the arms, we can do the trunk or body and we can do that in a circumferential fashion.

And that’s one of the key things that I think we need to determine during your consultation is some people just point here but we really need to do … point to their abdomen but we really need to do the waist and the back. And when I say the back, sometimes it’s all the way up and down, sometimes it’s just the waist that’s low down because that area … If we’re going to treat you circumferentially, we’re going to get a much better result than just spot treating one area. And we are going to topographically, just like a map, identify all the areas that bother you and address those in, usually, a circumferential fashion to help treat all of the areas to give you a very harmonious result, not like we’ve just spot treated an area. We really want to treat the whole person, not just one little area.

What You Should Know About Vaser® Liposuction

VASER liposuction is beneficial not only for excess fat deposit removal but also for shrinking the skin better than traditional liposuction. Because of the high-frequency energy it delivers, there is typically less bruising and swelling. For patient safety, we limit the removal of fat to 5 liters.

Liposuction may be used to reduce fat deposits to the following areas:

  • Neck
  • Abdomen and waist
  • Upper arms
  • Hips and buttocks
  • Breast or chest area
  • Thighs (inner and outer)
  • Back
  • Inner knees

You will be placed in a compression garment following surgery. Liposuction procedure is done on an outpatient basis, takes 1-3 hours to perform depending on the number of sites being treated and is done under general anesthesia.

After Liposuction Surgery

During the first 24-48 hours following surgery, excess liposuction fluid and liquefying fat cells drain from the small skin openings. The day following surgery, you may shower. You should wear the compression garment 23 of the 24 hours of the day including to bed at night for the first week.

Beginning week two, a spandex-type garment that applies a lesser amount of pressure is recommended to wear at least 12 hours a day for an additional 2-3 weeks of compression. The goal is to train the skin so it shrinks and re-drapes over the areas of fat removal and adheres at its new position. You may return to work in 1 week following your liposuction procedure.

So, the morning of your liposuction procedure, we’ll have you at the surgery center. It’s about a hundred yards from my office. It’s a licensed, accredited outpatient surgery center. We’ll have you meet with the anesthesia staff and the nurses. They’ll get you all ready. Then I will come in and draw on you. We will stand you up, and we typically use two or three different colored pens to mark areas we call the zone of adherence. Then we’ll mark areas that may have little divots or dents that we may want to try to shift fat into, and then we’ll topographically mark with kind of a topographic map, circles, the areas you want to treat. And once we have those all marked off, we’ll get you into the operating room.

We will oftentimes circumferentially prep you, meaning you’ll be cleaned 360 degrees around each body part that we’re going to work on. So, if we’re going to work on the front and back, we will clean the whole area. We typically will start on the back side, and then finish that, then flip to one side or the other side, or sometime just flip back onto the back. And we’ll treat all the areas circumferentially.

And the incisions are very small. We try to hide them in inconspicuous places. So if someone has a little scar from a prior mole removal, I’ll use that spot. I try not to set the incisions up like a ladder, where they’re perfectly symmetric. I try to put one a little high, one a little low, and that’s trying to throw off people, so if someone were to look at you, it doesn’t want to look like you have a bunch of incisions that were placed there for liposuction. We can try to camouflage them and make it look like it’s natural. And again, we don’t want to have those incisions placed somewhere where they can be seen. We try to hide them, make them very minuscule.

And after surgery we will wrap you up in what’s called a garment, a compression garment. So there’s typically two types of garments: phase one, phase two. The first garment we’ll place on you, it has clasps, hooks, zippers, so it’s easy for us to get on in the operating room, which is important because you’re asleep. And you’ll wear that essentially for one week, 24 hours a day, take it off an hour a day to shower. The second week through the fourth week, we’ll have you in a stage two garment, which essentially is like a Spanx shapewear garment. Even Under Armour makes some great spandex products, and we sell some, we have them for you, and we can just pick the size that works for you. Some people will buy their own. And you will wear that product generally 12 hours a day, 10 to 12 hours a day you’ll wear that. Generally, during the day you will wear it, and then you won’t wear it at night when you go to bed. And that helps train the skin where to go, where to shrink, how to compress, and ultimately gives you the shape you’re looking for.
